Job Purpose
Collaborate with multi-disciplinary teams to provide the highest quality of home-based palliative care and nursing care to terminally-ill cancer patients .
To effectively and creatively delivery total care to the patients, with instructions to manage pain and symptoms, and assist the patients and families manage death with dignity.

Educational Qualifications

• 2-8 years of documented nursing experience with a registered medical institute

Role Requirements

Key Responsibilities (including, but not limited to)
• Provide home-based palliative care to cancer patients in Dehradun as an integral member of the Ganga Prem Hospice Home Care Services Team
• Adhere to all Hospice policies, nursing protocols and code of conduct
• Continuously improve patient care standards through ongoing development of nursing team
• Evaluate the work and performance of juniors and assigned staff periodically
• Document and report any accident or incident in which you are involved
• Perform extra duties when called upon especially in exigent circumstances
• Be collaborative and cooperative with inter-disciplinary teams (counsellors, social workers, volunteers and Management) to plan and give patient care
• Make frequent visits to the patients to evaluate their condition. Palliate the patient's condition as required, and document each visit in the hospice format(s) real-time
• Get oriented to all services offered by GPH India, including all related protocols, rules and routines
• Observe and interpret the patient's symptoms and communicate them to the palliative care doctor(s)
• Accompany and assist the doctor(s) in rounds, dressings and any procedures as instructed
• Prepare nursing care plans and render effective bedside nursing case (including sponge bath, hair care etc.)
• Coordinate with palliative care doctor, Nursing In-charge (IC) and administrative assistant(s) during patient admission, care, movement and discharge or death
• Ensure that the 'Patient Care Plan' (PCP) and related data are kept up to date and 100% accurate
• Assist the patient with physical therapy and exercises routinely
• Ensure that the patient is receiving correct and good quality (hygiene + taste) diet
• Maintain personal, patient, Hospice and environmental hygiene
• Check and record vital statistics and signs legibly and promptly
• Be aware of and implement protocols across the Hospice services (Home Care, In-patient Care, Cancer Camps and Bereavement Support) such as admission, care, discharge, death, hygiene, infection control, waste management, patient safety, patient and staff education etc.
• Handle with care and with knowledge all Hospice gadgets, instruments and equipment used for patient care in the wards / elsewhere
• Handle waste segregation and management and disposal as per Policy
• Follow strictly all safety measures involved in patient care and self-protection as per Policy
• Attend programmes and training sessions (Management-proposed and self-initiated) to develop own knowledge and career
• Ensure strict discipline and decorum at all times, including that of nursing assistants and other junior staff
• Ensure proper and smooth handover and takeover during the change of rostered shifts
• Be aware of patients' rights and preserve the same
• Avoid any violation of confidentiality of patient and Hospice information / data. For instance, no discussion of patient identifiable information in public spaces
• Be aware of and implement global palliative care standards at the Hospice
• Assist in feeding of the patient, as required, including NJ / FJ feedings
• Assist the patient in their toilet, dressing and cleaning requirements
• Participate in all quality improvement activities of the Hospice
• Assist in shifting, accompanying and transporting of patients as required (example to-and-fro from hospice to home / hospital)
• Report any incidents or vital abnormalities to the CMO and palliative care doctor(s)
• Assist the Management in the patient referral system and spreading awareness thereof. Visit hospitals and oncologists to get referrals as instructed by the CMO / palliative care doctors
• Help the patient and family / caretaker cope with the upcoming loss
• Ensure that medicine consumption / usage records are kept up to date and accurate
• Take increasing initiative inpatient and staff recreational activities - by organising as well as participating
• Get continuously trained to improve at the use of email, MS Office, AI and other Hospice systems
• Ensure compassionate communication with patients, families and Hospice colleagues
• Be totally clear on the medication dose(s) to be administered. In case you do not understand, clarify from the doctor(s) immediately
• Keep the family updated through in-person or telephonic conversations in case of deterioration in patient's condition
• Safeguard all Hospice property, equipment and infrastructure at all times as per Policy
• Remain mindful and respectful, especially with colleagues and seniors

• Total responsibility of medicine (especially opioids) consumption recording and reporting. Efficient coordination and reconciliation with Pharmacy team
• Contribution toward supervision of staff, mentoring and training of junior staff and recreational efforts for the Hospice (training & development)
• Complete responsibility of quality and accuracy of data and documents
• Proper consultation with CMO, palliative care doctors and Nursing IC prior to and after commencement of care
• Improvement in service and professional standards of nursing assistants and interns
• Willingness and ability to work with multiple responsibilities at the same time and on a roster / rotational basis between HCV, IPD and other services of GPH India
