We are seeking a compassionate and dedicated Home Health Nurse to join our team at Our family Clinic . The ideal candidate will provide high-quality, patient-centered nursing care to individuals in their homes, ensuring they receive the necessary medical attention and support in a comfortable and familiar environment. As part of our multidisciplinary team, you will collaborate with other healthcare professionals to optimize patient outcomes and promote overall well-being.
Key Responsibilities:
• Provide direct, patient-focused nursing care in patients' homes, adhering to established care plans.
• Perform assessments of patients' health conditions, including vital signs, medication management, wound care, and other nursing tasks.
• Administer prescribed treatments and medications, ensuring accurate and safe administration.
• Educate patients and their families on health management, disease prevention, and self-care techniques.
• Monitor and document patient progress, making recommendations for adjustments to care plans as necessary.
• Collaborate with physicians, social workers, and other healthcare professionals to coordinate and improve patient care.
• Provide emotional support to patients and families, ensuring a holistic approach to care.
• Ensure compliance with healthcare regulations, infection control practices, and the organization's policies and procedures.
• Manage patient records and ensure accurate and timely documentation.
• Participate in periodic evaluations of patient care services to ensure quality standards are met.
Qualifications:
• Registered Nurse (RN) with a current and valid state nursing license.
• Experience in home health care or clinical nursing preferred.
• Strong assessment, critical thinking, and problem-solving skills.
• Ability to work independently, manage time effectively, and prioritize tasks.
• Compassionate, patient, and respectful of diverse backgrounds and needs.
• Excellent communication and interpersonal skills.
• Knowledge of home health regulations and standards of practice.
• Reliable transportation and a valid driver's license (for home visits).
Preferred Skills:
• GNM/BSC /ANM
