Job Description

Key Responsibilities:

1. Educating patients and families about various treatment options and answering any questions they may have.

2. Evaluating living donors and recipients and coordinating post- transplant follow-up.

3. Plans and coordinates in-hospital transplant services, solicits organ donors.

4. Communicates with donors, patients, and health team members to ensure comprehensive documentation, including informed consent of potential organ donor and transplant recipient

5. Coordinates in-hospital services and counsels recipient and donor to alleviate anxieties and assist recipient and donor throughout procedure.

6. Will act as SPOC for the patient and the donor or donor's family.

7. To facilitate the process of organ donation and transplantation.

8. Coordinates evaluation process for pre-transplant (referral) patients to ensure effective use of hospital and clinical resources.

9. Facilitates the scheduling of studies or procedures for diagnostic purposes utilizing fiscal responsibility for patient and hospital costs.

10. Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team.

11. Provides on-call coverage, including coordination of activities prior to transplant, and as medical triage for out patients in collaboration with the physician on-call.

12. Coordinates transplantation process when a donor is available (24 hours/7 days a week) including communication between physicians, local organ procurement organization, patient/families and the medical team involved including ED, OR, ICU (SICU, ACU), blood bank and histocompatilbility labs.

13. Completes necessary documentation according to the guidelines including listing, status changes and post-transplant follow-up information.

14. Communicates with multidisciplinary team members and patients and families in a timely manner in regard to patient care/issues.

15. Collaborates with other team members in coordinating discharge plans for patients pre-transplant or post-transplant including referral to home health / IV therapy / Hospice / Rehab / SNF.

16. Communicates with insurance care manager on an as needed basis.

17. Maintains patient transplant record with current data to facilitate communication.

18. Educates patients along the continuum to assist understanding of processes and procedures surrounding pre- and post-transplant issues.

19. Develops patient education materials or identifies external resources that are appropriate for patient education in all phases of transplantation care.

20. Communicates effectively to maintain positive working relationships and achieve desired outcomes.

21. Participates in unit, departmental and hospital-wide committees, task forces or meetings as indicated by role appropriateness.

22. Participates in organ transplant program planning and management within the health system .

23. Responsible for the smooth functioning of dialysis department.

24. Will act as Secretary to the department of Nephrology and Urology

25. Any other duties as assigned by the Management from time to time.

Job Types: Full-time, Permanent

Pay: ?20,000.00 - ?40,000.00 per month
