Job Description

Psychosocial assessments:

Conducting comprehensive evaluations of clients' mental health needs, including their social, emotional, and environmental factors impacting their wellbeing.

Individual and group therapy:

Providing counseling and therapeutic interventions to individuals, families, or groups to address mental health challenges.

Treatment planning:

Collaborating with the treatment team to develop individualized treatment plans based on client needs and goals.

Crisis intervention:

Providing immediate support and intervention during mental health crises.

Discharge planning:

Coordinating discharge plans from inpatient settings, including securing housing, linking clients to community services, and arranging follow-up appointments.

Community resource linkage:

Identifying and connecting clients with relevant community resources like housing assistance, support groups, and employment services.

Collaboration with healthcare team:

Working closely with psychiatrists, psychologists, and other healthcare professionals to ensure coordinated care.

Client advocacy:

Acting as an advocate for clients by navigating systems and advocating for their needs.

Documentation and reporting:

Maintaining accurate and detailed client records, including progress notes and treatment plans.
