Key Responsibilities:1. Medication Management:
• Prepare, dispense, and distribute medications in accordance with legal, ethical, and institutional guidelines.
• Review and interpret physician orders, ensuring accuracy and appropriateness of prescribed medications.
• Monitor patient medication therapies for efficacy and safety, making recommendations for adjustments as necessary.
• Monitor patient outcomes and adverse drug reactions, reporting and managing any issues that arise
• Conduct medication reconciliation upon patient admission, transfer, and discharge.
• Provide drug information and education to medical staff, patients, and caregivers.
• Manage the hospital's medication inventory, ensuring appropriate stock levels and proper storage conditions.
• Coordinate with suppliers and manage procurement processes.
• Ensure compliance with regulatory requirements for medication storage and handling.
• Maintain accurate records of medication dispensation and usage.
• Participate in hospital accreditation processes and ensure adherence to institutional policies and procedures.
