They play a crucial role in patient care by managing medication therapy and promoting safe and effective drug use. Key Responsibilities: Medication Dispensing: Review and process medication orders, ensuring accuracy and appropriateness of prescribed therapies. Prepare and dispense prescribed medications, following dosage and administration guidelines. Patient Education: Provide medication counseling to patients, explaining usage, dosage, potential side effects, and precautions. Address any questions or concerns patients may have about their medications. Drug Information and Consultation: Offer drug information and clinical support to healthcare professionals, including physicians, nurses, and other medical staff. Collaborate with healthcare providers to optimize medication therapy plans. Medication Management: Monitor patient medication profiles to identify potential drug interactions, allergies, or contraindications. Make recommendations for dosage adjustments or alternative medications when necessary. Regulatory Compliance: Ensure compliance with all relevant laws, regulations, and policies governing the practice of pharmacy. Keep up-to-date with changes in pharmacy laws and regulations. Inventory Management: Oversee medication inventory, including ordering, receiving, and maintaining appropriate stock levels. Monitor expiration dates and manage the disposal of expired or unusable medications. Quality Assurance: Participate in quality improvement initiatives related to medication management and patient safety. Conduct periodic audits and assessments to ensure compliance with quality standards. Documentation and Record Keeping: Maintain accurate and up-to-date patient records, including medication profiles and relevant health information. Document interventions, recommendations, and other pertinent information. Qualifications: Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program. Valid state pharmacy license and in good standing with the relevant pharmacy board. years of experience in a pharmacy setting; hospital or clinical experience is a plus. Strong knowledge of pharmaceuticals, drug interactions, and therapeutic guidelines. Proficiency in pharmacy management software and electronic health record (EHR) systems.
