Key Responsibilities:
• Patient Support and Advocacy:
• Provide empathetic, non-judgmental support to patients and their families.
• Serve as a patient advocate to address concerns, preferences, and needs within the healthcare system.
• Information and Education:
• Explain medical diagnoses, treatments, and procedures in an understandable manner.
• Ensure patients fully understand their care plans and medication instructions.
• Educate patients on lifestyle changes, preventive care, and self-management techniques.
• Emotional and Psychological Support:
• Offer counseling to patients dealing with chronic illnesses, trauma, or stress related to their health conditions.
• Provide grief and loss support to patients and families when necessary.
• Coordination and Resource Navigation:
• Assist patients in scheduling appointments, obtaining referrals, and accessing healthcare services.
• Connect patients with community resources, financial aid programs, or support groups.
• Compliance and Follow-Up:
• Monitor patient adherence to treatment plans and encourage active participation in their healthcare.
• Conduct follow-ups to ensure continuity of care and address any emerging concerns.
Qualifications:
• Bachelor's degree in counseling, psychology, social work, or a related field (Master's degree preferred).
• Certification in counseling or patient advocacy is a plus.
• Strong knowledge of healthcare systems and medical terminology.
