HEAPS is looking for empathetic care coach who will be dedicated and passionate to monitor patients with chronic conditions using the HEAPS AI driven care management platform.
As a Care Coach for Chronic Care Management, you will play a pivotal role in enhancing the well-being of patients with chronic conditions. Your primary responsibility will be to monitor and track the health progress of patients, while providing them with guidance, support, and encouragement to follow their personalized care plans effectively. By ensuring compliance with prescribed treatments, medications, and lifestyle modifications, you will contribute to better disease management and improved health outcomes for patients.
Responsibilities
Engage with the patients over various touchpoint calls to understand their current medical health status, past medical/surgical history, medications taken, labs done etc.
• Understand if the patient has any additional requirements - medications, labs, dietitian, physiotherapists etc.
• Understand patients' current social determinants and lifestyle status
• To effectively engage the patients in giving complete medical history and determine if they have any deviations from the recommended care plan.
• Provide patients with the psychosocial support needed to cope with chronic, acute, or terminal illnesses.
• Monitor patients to evaluate the progress of their health
• To smartly handle patient queries and provide responses from pre-defined FAQs.
• Educate patients on their chronic conditions, treatment options, and the importance of consistent adherence to their care plans
• Understand if the patient is compliant to Labs, Medications, and Diet & coach them effectively in order to be compliant with the prescribed treatment plan
• To ensure effective patient data management
Skills
Excellent knowledge and interpretation of medical conditions and medical terminologies
• Effective Patient medical history taking skill
• Coaching the patient to be compliant with different medical parameters
• Clarity in communication; Ability to articulate and talk to the patient in a clear manner without ambiguity
• Active Listening skills, being compassionate with the patients
• Passionate about the role and have patient care as a priority
• A natural conversationalist: Should be well-read and have the ability to engage patients on variety of topics
• Handling complaints and resolving grievances and conflicts which the patients might have
• Entering, transcribing, recording, storing, or maintaining information in written or electronic form
• Analytical mindset with the ability to interpret health data and trends effectively
• Pleasant disposition, naturally pleasing to talk and have positive attitude
• Collaboration is key; should be a team player and can bridge the gap across all departments.
Qualifications
Minimum bachelor's degree in clinical/Medico/Biology background
(Preferred Pharmacists, Registered Nurses, Clinical Psychologists, Dietitian, Nutritionist, or such related fields)
• Proficiency in Telugu,English and Hindi
• Minimum 1 to 2 years' experience in wellness coaching/health coaching or care management process
