### Position Summary:



The Outreach Worker will be responsible for engaging with and supporting individuals experiencing homelessness in the community. This position involves field visits, providing direct care and support, and ensuring access to critical services such as medical care, hygiene assistance, and social support. The ideal candidate will have a strong desire to help vulnerable populations, excellent interpersonal skills, and the ability to manage complex situations in the field.



### Key Responsibilities:



• Engagement with Beneficiaries:



+ Establish trust-based relationships with homeless individuals through outreach efforts in the community.
+ Identify and engage individuals in need of support and assistance.
+ Provide information on available services and guide individuals in accessing them.
• Hygiene Support:



+ Ensure beneficiaries have access to basic hygiene products and services.
+ Educate individuals on the importance of personal hygiene for health and safety.
+ Coordinate access to showers, clean clothing, and hygiene kits where possible.
• Field Visits:



+ Conduct regular outreach visits to designated areas to assess the needs of homeless individuals.
+ Monitor the well-being of individuals and provide immediate support when necessary.
+ Identify and respond to urgent situations, including medical or safety concerns.
• Assistance with Hospitalization and Medical Care:



+ Help individuals access medical care, including assisting with hospital admissions and providing transportation to healthcare facilities.
+ Follow up on hospitalization cases to ensure that individuals are receiving the necessary care.
+ Provide or coordinate basic on-the-street medical support for minor injuries or illnesses.
• On-Street Medical Care:



+ Provide first aid and other basic medical care, if trained and within scope, in emergency situations.
+ Work in collaboration with healthcare professionals to meet medical needs of homeless individuals.
+ Help facilitate connections with medical professionals and services.
• Community Care and Service Coordination:



+ Develop and coordinate community care services to meet the health, social, and emotional needs of the homeless population.
+ Connect individuals with essential services such as shelters, food programs, mental health support, and substance abuse treatment.
+ Collaborate with local service providers, including healthcare organizations and social service agencies, to ensure holistic support for clients.
• Follow-Up and Activity Tracking:



+ Maintain detailed records of all outreach activities, interventions, and follow-up tasks.
+ Track progress and outcomes for each individual case, ensuring that beneficiaries are receiving the necessary support.
+ Prepare regular reports on outreach efforts, client status, and service utilization.
