Clinical Pharmacist (Chronic Disease Management & Medication Therapy Management)
Role:
Play a pivotal role in optimizing medication therapy for patients with chronic diseases, improving health outcomes, and reducing healthcare costs through a Medication Therapy Management (MTM) program.
Responsibilities:
• Conduct comprehensive medication reviews: Analyze patients' medication regimens, identifying potential drug interactions, duplication, and adherence issues.
• Develop and implement personalized MTM plans: Tailor medication therapy to each patient's specific needs, considering disease state, comorbidities, allergies, and socioeconomic factors.
• Educate and counsel patients: Provide clear and concise information about medications, including proper usage, side effects, and potential interactions, health, and lifestyle.
• Monitor and track medication adherence: Implement strategies to improve adherence, such as medication reminders, education, and support groups.
• Collaborate with healthcare providers: Work closely with consultants, physicians, nurses, and other healthcare professionals to ensure coordinated care and optimal medication therapy.
• Perform data analysis and program evaluation: Track and analyze data to assess the effectiveness of the MTM program and identify areas for improvement.
• Stay up to date on the latest clinical guidelines and research: Continuously expand knowledge base to provide patients with the most effective and evidence-based care.
Qualifications:
• Doctor of Pharmacy (Pharm.D.) degree
• Current and valid pharmacy license
• Strong clinical knowledge and experience in chronic disease management, particularly in the specific diseases targeted by the MTM program
• Excellent communication, interpersonal, and collaboration skills
• Patient-centered and results-oriented approach.
• Proficient in electronic health records (EHR) systems.
