Responsible for the areas of care management and assists patients and families in understanding patient conditions and treatments.
Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies, and others.
Acts as the first point of contact for the patient for all needs and will be responsible for coordinating both internally and externally to the hospital to ensure their needs are met.
Develops working relationship with leaders in community resource groups, representing the agency at meetings and on various committees that address the social needs of patients such as NGOs, etc. Provides general overview on treatment options to patients and helps coordinate services needed after discharge, such as medical equipment, physiotherapy, dietary needs, homecare services, etc. Adheres to department/hospital policies.
Helps in connecting patients with services such as accommodation for family members or transport to and from the hospital.
MSW will be the custodian of all patient communication and feedback. He / she will track all responses and actions taken in response to patient complaints/issues/queries.
Coordinate with doctors and nursing team for patient calls where doctor intervention is needed and maintain log of the same.
Helps patients and family members develop coping strategies; Medical social workers also arrange grief counseling or other mental health services for patients when requested.
Plans events for cancer patients and motivational programs as outlined in the annual calendar to include patient engagement activities such as meditation classes, music or art therapy, motivational talks by survivors, etc.
Partakes in the spreading of cancer awareness in marketing activities.
