Home Care RMO

to join our team. The Home Care RMO will be responsible for coordinating home care visits, ensuring that patient care continues after hospitalization, and working closely with the Home Care Incharge. This role involves visiting patients at their homes to provide medical care, monitoring their recovery, and ensuring smooth coordination between patients, their families, and the hospital's medical team.

Key Responsibilities:

Coordinate with the Home Care Incharge to arrange and schedule home visits for post-discharge patients. Act as the primary point of contact between patients and the hospital for home care-related queries. Visit patients at their homes to provide medical care, follow-up treatment, and monitor their recovery. Ensure all required medical supplies, equipment, and staff are coordinated for each home visit. Maintain detailed documentation of home visits, including patient conditions, progress, and treatment plans. Collaborate with the hospital's medical team to ensure appropriate follow-up and continuity of care. Address any concerns or issues raised by patients or families during home visits and ensure timely resolution.

Eligibility Criteria:

Minimum 1 year of experience as an RMO or in a similar role within a multi-specialty hospital. Strong coordination and communication skills to effectively work with the medical team, patients, and family members. Knowledge of post-discharge care and home care procedures. Compassionate and patient-centered approach to care. Ability to manage multiple home visits efficiently.

Qualifications:

BHMS, BAMS, MBBS
