Home Care Incharge

to join our team. The Home Care Incharge will coordinate home care visits, ensuring smooth communication between patients and the hospital. The role involves collaborating with RMOs to arrange and manage home visits for post-hospitalization care.

Key Responsibilities:

Coordinate and manage home care visits, ensuring patients receive the necessary care at home. Work closely with RMOs to schedule home care visits and ensure appropriate follow-up care. Act as the primary point of contact between patients and the hospital for home care-related queries. Ensure coordination of medical supplies, equipment, and staff for home visits. Oversee planning and logistics for home care services to ensure timely and efficient delivery. Maintain accurate documentation of all home care visits. Monitor patient progress during home visits and communicate with the medical team for continued treatment. Handle patient complaints or concerns regarding home care services and ensure timely resolution.

Eligibility Criteria:

Minimum 2 years of experience in a similar home care role in a multi-specialty hospital. Bachelor's or Master's Degree in a relevant field. Strong communication, organizational, and coordination skills. Ability to work effectively with medical teams to manage patient care outside the hospital. Knowledge of home care procedures, patient safety, and post-discharge care. Ability to manage multiple tasks and home visits efficiently. Compassionate and patient-centered approach.
