Unit Specific responsibilities
• To handle patients of all ages.
• Overseeing the administrative functions of the unit.
• Responsible for daily deployment of staff according to the bed occupancy & nurse ratio.
• To monitor the pain assessment of the patient is done periodically.
• Responsible to provide department and unit specific orientation on policies including biomedical equipments, resuscitative techniques etc to the staff.
• To monitor and ensure the appropriate implementation of all hospital and department specific policies and procedures.
• Assess the physical condition of the patient.
• To act as per the defined role on the activation of the emergency codes.
• Responsible to impart trainings to the staff.
• To plan the patient care according to patient's condition.
• Early & timely assessment of the patient to detect any complications.
• To monitor taking over patients at the patient's bed side and the details of the ongoing treatment from previous shift nurse properly.
• To monitor that the emergency medical supplies are readily available in the ward and periodically monitor their expiry date.
• To monitor taking of stock of the drugs in coordination with Pharmacist and note the same in the respective registers.
• To monitor that all equipments in the ward are in working condition and to coordinate with biomedical department in case the equipments are not in proper working condition.
• To monitor availability of sterile dressing packs in the ward.
• To monitor availability of stationery (Consent forms, nurse notes, discharge summary etc) in the ward.
• To monitor that any untoward incident is notified following the process of incident reporting.
• Ensure safety and emergency preparedness.
• To promote professional development through resuscitation technique like BLS, ACLS.
• Follow infection control & various hospital policies.
Performs Ongoing Patient Assessment
• Complete timely nursing assessment & perform ongoing assessment.
• To monitor and collect patient information and review the plan of care.
• To monitor and obtain vital signs, haemodynamic values and neurological values.
• To monitor that any reportable information related to the patient is notified to the treating physician.
Perform patient physical care
To monitor and provide Neuro, cardiovascular, musculoskeletal, endocrine, renal, back care and assessment of hemodynamic status of the patients as required.
To monitor, assess and identify vascular access needs and maintain fluid and electrolyte balance.
To monitor and ensure utmost comfort and provide personal hygiene.
To monitor and provide nutritional support and promote wound healing.
To monitor and collect patient blood samples and send it to the appropriate labs for investigations.
To monitor and provide patient care during transport.
Patient medication
To monitor safe drug administration and monitor the patient.
To monitor and ensure correct and timely medication is provided to the patient.
To monitor indents and return of medicines.
To monitor complete medication i.e. administer IV Medications via central lines and peripheral lines and ensure correct titration of medications if required.
To monitor and administer oral, intramuscular and subcutaneous medication.
Equipment upkeep
To monitor the machine regularly during dialysis.
To monitor sterilization, storage and reuse of dialyzers.
To monitor disinfection of machine.
Check dialysis tubing's and dialyzer and should be stored in a clean place.
To monitor, obtain and setup appropriate equipment and supplies.
To monitor optimum utilization and upkeep of unit specific equipments i.e. dialysis machine and glucometer.
To monitor daily check of crash cart for seal.
To monitor analyzing of equipment related patient result and response, troubleshooting unexpected findings, solving the identified problems, evaluating need for continual use of equipment, and discontinuing the use of equipment if required.
Assist in specialized procedures
• Monitoring the patient during blood transfusion.
• To monitor and provide care of fistula, grafts, internal juglar catheter.
• To perform CPR and medication in assistance with the doctor during code blue situation.
• To assist the doctor during Intubation, extubation, pulmonary artery catheter insertion and elective cardio version.
• Assist in procedures like insertion of lines, insertion of ryle's tube, catheterization, paracenthesis, nuerological patients.
Patient & family Education
To maintain the confidentiality of patient.
To monitor and provide patient & family education.
To monitor diagnosis specific patient and family education is provided to the patient and his attendants.
To act as a team member during crisis management and participate in end of life care including grief management.
Maintain safe environment
To monitor and provide safe environment for staff and patient.
To monitor and protect patient from injury, ensure accessibility to emergency medical equipment, ensure infection control protocols are followed, correct biomedical and hazardous waste disposal, maintain and ensure electrical and fire safety.
Active participation during code HAZMAT situation.
Documentation
To monitor maintenance of dialysis records and intra dialytic events.
To monitor correct unit specific documentation especially handing and taking over, maintenance of critical care flow sheet and reporting & recording of information.
Responsible to document unusual occurrences, patient transfers and quality control parameters.
To monitor and maintain unit specific log book and registers.
? To perform all other jobs as may be assigned due to exigencies of work.
? Participation in Continuous Quality Improvement.
? To follow the Process Improvement programs and carry on Quality Initiatives in the department.
