• Gather pertinent information about the patient's medical and family history- record information in the form provided.
• Coordinate with the counselors and patients to assist in organizing appointments while closely working with the patient's caregivers and doctor's/hospital's team.
• Assist cancer patients/caregivers with accessing all the available resources for cancer patients that are provided by the doctor/hospital.
• work with the central team to provide counseling support and community program support to patients based on their needs.
• Provide emotional support to the patients as needed.
• Be an advocate for the patient.
